A pergunta principal aqui pode ser: Câncer é só uma questão de azar genético ou será que talvez nós estejamos causando esse problema através do nosso estilo de vida e hábitos alimentares? O meu convidado de hoje é o Dr. Thomas Seyfried do Boston College e autor do livro “Cancer as a Metabolic Disease”. Ele acredita que câncer não é uma doença genética, mas sim uma doença metabólica e mitocondrial – você entenderá mais sobre isso na nossa conversa de hoje. Boa tarde, Professor. É uma honra tê-lo aqui hoje. Como você está?

Thomas Seyfried: Muito obrigado, Rodrigo. É um prazer estar no seu show e ter a oportunidade de discutir algumas ideias sobre a origem, prevenção e gerenciamento do câncer.

Rodrigo Polesso: É um assunto muito grande. Não vejo muita informação de qualidade sobre isso no mundo, e muito menos no Brasil. Então, é muito bom termos essa conversa hoje. Dados da American Cancer Society mostra que mais de 1600 pessoas morrem de câncer por dia nos EUA. Isso está correto?

Thomas Seyfried: Esses são os números que eles fornecem. Todos os anos eles mostram o resultado geral do problema do câncer. Eu cataloguei esses números ao longo dos anos. O interessante é que o número de novos casos tem aumentado dramaticamente nos EUA. Eu acho que o mesmo acontece nos outros países ocidentais. Eu acho que isso é em parte por causa da tecnologia para identificar o câncer. Mas o mais problemático é o aumento no número de pessoas que morrem por ano. Se você analisar os dados de 4 anos atrás, o aumento na taxa de morte é mais rápido do que o aumento de novos casos – em números relativos, não em números de pessoas. Isso é problemático, já que isso mostra que não estamos fazendo progresso no gerenciamento da doença. As pessoas podem debater isso, mas o número de mortes fala mais alto. Se houvesse uma redução nos casos de câncer, ou um novo tratamento poderoso, nós veríamos uma grande queda no número de mortes, muito similar ao que estamos vendo com a AIDS. Antes dos coquetéis antirretrovirais, a AIDS era letal. As pessoas estavam morrendo em grandes números até que terapia foi desenvolvida. Ela não cura a doença, mas causou uma grande queda no número de mortes. Não vemos nada parecido com o câncer. Até que nós comecemos algo do tipo, temos que ficarmos alertas com o que está acontecendo. O que estamos fazendo com essas pesquisas, dinheiro e investimento? Obviamente, algo está muito errado.

Rodrigo Polesso: Sim. Isso faz bastante sentido. Nós normalmente falamos sobre obesidade aqui. Eu acho que a mesma coisa acontece. Nós vemos novas dietas, tratamentos e remédios sendo lançados, mas não vemos quedas nas taxas de doença metabólica. Eu acho que o mesmo acontece com o câncer. O paradigma principal diz que câncer é uma doença genética e um problema de sorte. O novo paradigma que você lidera é que o câncer é uma doença mitocondrial metabólica. Eu acho que isso é muito interessante e é algo difícil de ser ignorado. Não é genético na maioria dos casos. Na verdade, é causado por nossa dieta e estilo de vida. O que você poderia nos dizer sobre isso?

Thomas Seyfried: Exatamente. Temos sido levados a um caminho de informações erradas por décadas. Temos que usar a perspectiva histórica para entender o que levou o câncer ser considerado uma doença genética. Depois da Segunda Guerra, existiu muitos trabalhos sobre os cânceres oriundos de vírus. Muitas perguntas surgiram: como essa doença surge? É de um agente infecioso? É um vírus? Eu acho que a estrutura de DNA teve um grande impacto na mentalidade da ciência e medicina da época. Foi quase como uma tranqüilização da mente. As pessoas adoraram isso. Se você analisar a expressão genética do câncer, é claro que ela será anormal. Quase todo o campo científico está tentando levar a medicina e biológica para o nível molecular. Existe uma “inveja” dos físicos há séculos. Os físicos sempre foram reconhecidos como os estudiosos do mundo natural. Já a biologia sempre foi vaga e de difícil definição. Existem variáveis demais nela e nada é quantitativo. Os bioquímicos estavam entrando no nível microscópico. Mas quando você encontra mutações no nível molecular que talvez sejam importantes… É algo tão emocionante que é possível transformar esse campo ambíguo relacionado à medicina e câncer para entende-lo no nível molecular. Agora, ele se torna mais molecular, mais preciso, mais parecido com a física. Mas isso está muito longe da verdade quando se fala em câncer. Essa é uma doença humana conectada às nossas sociedades, às comidas que ingerimos e ao nosso estilo de vida. Tem muito mais a ver com o estilo da pessoa do que mudanças no nível molecular. Agora estamos vendo isso se transformar em uma abordagem industrial, onde as novas terapias são baseadas em mudanças moleculares e trazem grandes benefício financeiros àqueles que participam disso. As verbas de pesquisa são dadas aos indivíduos que estão pesquisando o câncer no nível molecular. Muito menos é dado aos comportamentos que causam a predisposição à doença ou às técnicas de nutrição para o gerenciamento da doença. Essas técnicas são menos quantificáveis, mas são extremamente poderosas para a abordagem da doença. Então, é um grande problema. Existe um grande mal entendido sobre a natureza da doença. Ela não é uma doença nuclear e genética. Ela é uma doença mitocondrial metabólica. Quando isso for compreendido nos níveis bioquímico, molecular e fisiológico, veremos uma mudança de direção e uma queda no número de mortes, assim como ocorreu com a AIDS.

Rodrigo Polesso: Sim. Parece que as pessoas insistem nos mecanismos das mutações. Nós sabemos que câncer tem vários tipos de mutações, mas elas têm uma coisa em comum: a maneira através da qual a célula trabalha com glicose. Toda as células cancerígenas não respiram direto. O mecanismo de respiração delas é “quebrado”. Então, elas fermentam para ter energia. Isso acontece em todas as células cancerígenas. É algo que todas elas têm em comum. Isso nos dá uma dica sobre o que é a causa disso.

Thomas Seyfried: Sim, isso é muito importante. Nos dá uma boa ideia de qual é o problema. Todas as células do tumor tem uma entidade genética diferente. Existem centenas de milhares de mutações. Existem mutações “drivers”, “passageiros”, “iniciadores”, “facilitadores”… Eles dão nomes para essas classes diferentes de mutações. Mas você está certo: todas essas células fermentam. A agressividade de um tumor está conectada a quanto ele fermenta. Os tumores mais agressivos são os que mais fermentam. Os que crescem mais vagarosamente são fermentadores parciais. Mas por que essas células do tumor fermentam? Se você for parte do dogma genético, você dirá que essas células fermentam porque elas têm mutações nos oncogenes. Por outro lado, se existe uma respiração danificada, essas células vão fermentar também. Sabemos que quando paramos de respirar o ácido lático se acumula no nosso corpo, já que nossas células começam a fermentar. Elas não conseguem oxigênio suficiente, então elas liberam ácido lático, assim como a célula de um tumor faz. A célula cancerígena está travada nesse estado de fermentação. Então, que tipo de mudanças genéticas poderiam acontecer para travar a célula nesse estado de fermentação? É aí que entram os oncogenes. Os oncogenes são os facilitadores do metabolismo da fermentação. Mas, se você permitir que essas células respirem, os oncogenes são desligados. Então, os oncogenes são um efeito, e não a causa da doença. Eles facilitam a habilidade da célula de ficar viva, já que não conseguem respirar. Eles são o efeito! Mesmo assim, esse simples conceito é impossível de ser compreendido pelo campo científico. Eles acham várias exceções que se baseiam em um sistema falho de experimentação para manter a ideia de que essa é uma doença genética. As pessoas novas que farão as mudanças, não os poderosos. Eles estão travados nisso. Eles estão travados no dogma. É muito difícil quebrar o dogma. Então, quando analiso o câncer como uma doença mitocondrial metabólica com soluções metabólicas, quem se beneficiaria disso? Os pacientes de câncer! Os pacientes iriam gostar disso. Os pacientes que colocarão pressão no establishment para que a mudança ocorra. A solução do establishment para esse problema não funciona. Ela é baseada em um conceito científico falho.

Rodrigo Polesso: É assustador como isso é similar ao que ocorre com a nutrição. Você provavelmente já sabe disso, é claro. O dogma é impossível de ser quebrado. Eu acho que demorará uma geração para a mudança começar. Quando os caras velhos começarem a sair de cena, a nova geração não pode mais ignorar isso. Eu acho que as pessoas estão conseguindo resultados com a nutrição e conseguirão resultados focando na causa do câncer. Como os resultados são reais, eles criarão uma demanda para isso. As pessoas não poderão mais ignorar isso. Para as pessoas que não conhecem esse processo de fermentação, é bom explicar isso. Estamos falando sobre alimentar as células cancerígenas. Você poderia explicar para as pessoas leigas como uma célula normal processa energia e como uma célula cancerígena faz isso? Só para as pessoas entenderem a diferença.

Thomas Seyfried: Eu acho que uma boa maneira de entender essa questão é pensar na origem da vida do planeta. Existiu um tempo quando não havia vida no planeta. Não havia nada. Gradualmente, os organismos biológicos apareceram há 2 ou 3 bilhões de anos. O primeiro tipo de organismo viviam num ambiente sem oxigênio. O oxigênio ainda chegaria ao ambiente. Mesmo assim, haviam seres vivos no planeta antes do oxigênio. Como essas células viviam sem oxigênio? Elas fermentavam. Elas usavam uma forma primitiva de metabolismo de energia. Elas não usavam o oxigênio para os elétrons, já que não havia oxigênio. Elas usavam outras moléculas como aminoácidos e proteínas primitivas. Todos os organismos faziam fermentação naquela época. A fermentação é o uso e desenvolvimento na ausência do oxigênio. Todos os organismos se proliferavam descontroladamente desde que existisse combustíveis fermentáveis no ambiente. Quando os combustíveis fermentáveis se tornaram limitados, esses organismos começaram a morrer. Quando o oxigênio passou a chegar no ambiente, há 1 bilhão e meio de anos, alguns organismos podiam capturar o oxigênio de maneira primitiva para obter energia. Então ocorreu uma cooperação entre esse organismo que capturava o oxigênio e esses que fermentavam. Ocorreu uma simbiose, como uma cooperação, entre o organismo que capturava o oxigênio e os organismos que fermentavam. A evolução mostrou que com o oxigênio você pode gerar muito mais energia, de maneira eficiente, sem usar tantos recursos. Nós gradualmente nos desenvolvemos de organismos unicelulares para pluricelulares. Com a eficiência em energia, essas células podiam fazer funções mais sofisticadas, o que possibilitou várias coisas. Ao invés de ter um organismo com somente uma célula, agora temos órgãos e tecidos. Todas essas células diferentes podem funcionar com eficiência em energia para conseguir feitos biológicos mais sofisticados. Agora existem mamíferos e outros organismos que se desenvolveram a partir desses organismos primitivos. No câncer ocorre um dano à essa relação simbiótica dentro da célula. Aquela bactéria que conseguia capturar o oxigênio emprestou muitos genes para o organismo que fermentava. Mas ela manteve 13 genes que são absolutamente críticos. Eles são os principais. Se algo acontecer com um desses 13 genes, a célula inteira morre. Mas eles foram mantidos na mitocôndria. A mitocôndria de hoje é o resultado desse organismo. Ela veio de uma bactéria. Ela permite fazer funções magníficas, facilitando a fisiologia do corpo. O que acontece é um dano à essa organela. Como o Warburg disse, se a organela e a função respiratória da célula está criticamente danificada, a célula morrerá. Ela não consegue se adaptar e ajustar à perda de energia. O câncer é o dano nessa capacidade respiratória. é um problema crônico. Não é um problema agudo. Ele permite que essa célula que usava a respiração de oxigênio passar para a fermentação usando moléculas orgânicas. Essa forma primitiva de respiração ainda está presente, desde antes do oxigênio surgir no planeta. Essas células voltam para o estado que existia antes do oxigênio, que era o estado de crescimento descontrolado, proliferação rápida e todo o resto que vemos na doença hoje. A solução para esse problema é ter como alvo os combustíveis fermentáveis que essas células usam e fazer com que o corpo usar combustíveis que as células cancerígenas não podem usar. É uma estratégia baseada na origem da doença e deve ser muito eficaz depois de termos a aprimorado.

Rodrigo Polesso: O principal combustível para a fermentação é a glicose, correto?

Thomas Seyfried: Glicose e outros aminoácidos. Muitos desses organismos de antes do oxigênio conseguiam fermentar carboidratos e aminoácidos. Então, temos que reconhecer que alguns aminoácidos podem ser fermentados também. Mas a grande parte dos combustíveis fermentáveis são a glicose e glutamina. Sem esses combustíveis as células cancerígenas não conseguem viver. Tendo esses dois combustíveis como alvo deve ser o suficiente para eliminar a maioria das células do câncer. Todos os tipos de tumores são iguais. Eles não são doenças diferentes – essa é uma concepção errada. Como você disse, o câncer vem em tecidos diferentes, ele tem mutações, mas é sempre fermentador. Não importa se é no cérebro, na bexiga, no peito… ele sempre estará fermentando. O que eles estão fermentando? Eles fermentam glicose e glutamina. A solução para o problema é se livrar da glicose e da glutamina de maneira inovadora e sofisticada. Quem está fazendo isso? Ninguém está fazendo isso.

Rodrigo Polesso: Algumas pessoas estão fazendo isso. Mas não numa escala grande. Eu vi alguns estudos em animais. Quando eles reduzem a glicose da comida, colocam pessoas em dietas cetogênicas, ou em jejuns prolongados… O suprimento de combustível dessas células é cortado. Uma célula humana normal sobreviveria sem glicose queimando cetonas e outras substâncias. Mas isso seria um problema para a célula cancerígena, correto? Se você cortar a glicose e a glutamina, o que acontece com as células do câncer? Elas “morrem de fome” ou ficam mais fracas? Algumas pessoas dizem que não tem como matar o câncer “de fome”.

Thomas Seyfried: Ainda não foi descoberto completamente. Estamos pesquisando isso nesse momento. Muitas pessoas estão tendo a glicose como alvo através de dietas cetogênicas e usando medicamentos para isso. Você tem total razão. O corpo pode trocar para um combustível alternativo: a cetona. Mas as células cancerígenas também podem trocar para a glutamina. Então, se você tem um tumor que depende da glutamina, você não vai conseguir benefícios através de dietas cetogênicas. Uma coisa que nós descobrimos é que a metástase – a proliferação do câncer pelo corpo – é a maior ameaça possível para quem tem câncer. Se você tem um tumor que não fez a metástase, um cirurgião pode facilmente curar esse câncer. Mas se as células desse tumor decidirem se espalhar, se torna mais difícil gerenciar essa doença. Que células são essas que deixam esse tumor primário e se espalham pelo corpo? Nós descobrimos que elas são parte do sistema imunológico. São macrófagos. O que os macrófagos comem? Elas comem glutamina! Elas usam glicose ou glutamina e vivem em ambientes hipóxicos. Esse conceito de terapias antiangiogênicas é superficial. As células já conseguem viver em ambientes hipóxicos. Então, terapias antiangiogênicas como Avastin não vão funcionar. Elas só têm um efeito superficial na doença. Quando você passa a entender a biologia da doença, você até consegue prever o que acontecerá. Daí você não precisa perder tanto tempo analisando as diferentes terapias para saber se vão funcionar ou não. Eles gastam milhões de dólares analisando essas drogas. Eu já sei se elas funcionarão ou não. Se elas são direcionadas ao metabolismo da célula, dá para saber logo do começo se elas funcionarão ou não. Existem vários medicamentos disponíveis que poderiam ter essas moléculas como alvo, mas eles não têm patente e as pessoas não os usam. Eles não são “emocionantes”. Para o paciente que tem câncer no pulmão, não importa se é “emocionante” ou não. Eles querem saber: “O que você tem para mim?” Eles também não querem esperar até que essas pessoas que estudam genética morrerem. Os pacientes estão lidando com uma crise. Eles querem algo para se manterem vivos. Eles não querem esperar 50 anos até que esses caras que estudam genética morrerem. Os pacientes vão acabar morrendo antes deles! É necessário entender a seriedade do que estamos falando aqui. Estamos lidando com as vidas de milhões de pessoas. Não precisamos esperar por um longo período de tempo, sendo que já temos as respostas. Só precisamos dos grandes laboratórios de pesquisa nisso. Eles procuram uma droga patenteável que possa fazer isso. Mas nós já temos drogas e procedimentos que podem fazer isso – o problema é que eles não conseguirão ganhar dinheiro com elas. O paciente não se importa com isso. Nós sempre precisamos pensar no paciente. O que paciente quer? O argumento é sempre esse: “Não existem testes clínicos para suportar essas terapias metabólicas.” Quem iria pagar por esses testes clínicos? Alguma grande indústria farmacêutica quer usar a terapia quando não existe uma droga patenteável nisso? Precisamos fazer experimentos double blind cross-over nisso tudo como se estivéssemos propondo o uso de uma droga tóxica. Isso não faz sentido. Temos que repensar a maneira de tratar pacientes com câncer. Em outras palavras: Nossa estratégia é chamada “Press Pulse Metabolic Therapy”. Nós colocamos os pacientes nesses diferentes estados metabólicos e usamos pulsos com drogas de baixa dose que funcionam bem com nutrição e dieta. Tudo isso funciona de maneira sinérgica. Os pacientes saem dessa terapia mais saudáveis do que quando começaram. Eles não precisam passar por essas anomalias médicas que são presentes nos tratamentos de câncer. Isso não faz sentido! Isso é loucura! Isso prova novamente o fracasso completo da abordagem à essa doença. Algo deve ser feito e deve ser feito logo. Por que precisamos esperar quando temos uma estratégia nas mãos?

Rodrigo Polesso: É difícil acreditar que isso acontece. Se o Ebola estivesse matando 1600 pessoas por dia, seria um caos mundial. Todo mundo estaria falando sobre isso. Não é o que acontece com o câncer. Não há muito que possamos fazer sobre isso, o que é uma pena. Quais resultados você vê nas pessoas que simplesmente implementam essas estratégias de maneira autônoma? Mesmo sem as drogas, mas só implementando um protocolo de jejum intermitente, jejum longo e dieta cetogênica. Você vê reduções, melhoras ou curas?

Thomas Seyfried: Nós não gostamos de usar o termo “cura”. Não tem como sabermos disso. O campo usa a marca de 5 anos. Tem muitas pessoas que vivem 5 anos e morrem depois de 6, 7 ou 8 anos por causa do câncer ou outro câncer que desenvolveram por causa do tratamento que eles receberam. Isso não faz sentido. Como você sabe se tem uma cura? Se você vive até envelhecer e morrer de outra coisa que não seja câncer. Somente aí você estaria curado do câncer. Como você gerencia essa doença, mantendo paciente saudável, com qualidade de vida, sem progressão da doença ou efeitos adversos do tratamento? É exatamente essa a nossa estratégia. Esse é o objetivo que queremos usar. A maioria dos nossos casos são “anedóticos”. Casos anedóticos são os casos que não passaram pelos testes clínicos, mas os pacientes aparentam estar muito saudáveis. Infelizmente, o establishment médico ao invés de ajudar os pacientes a se adaptarem à terapia metabólica, ele desencorajam os pacientes. Muitas vezes eles não apoiam as terapias metabólicas até que os pacientes já tenham passado pelo tratamento tóxico de cirurgia, radiação e produtos químicos venenosos. Depois do paciente ter fracassado nisso tudo, talvez eles considerem a possibilidade. A culpa não é inteiramente do nosso campo. Os médicos simplesmente não são treinados nas universidades para usarem comida como medicamentos. Eles não são treinados para usarem a fisiologia do metabolismo no tratamento do câncer. O câncer é visto como uma doença focal de um órgão ou tecido. Ele não é visto como uma doença sistêmica. Depois que você percebe que é uma doença sistêmica que pode envolver um seio ou um pulmão, você poderá tratar a fisiologia inteira do paciente. Eu acho que era algo raro. Talvez somente o câncer “real” e genético acontecesse no passado – o que era raro. É isso que vejo nos estudos – era uma doença rara. Hoje em dia é uma doença comum. Todo mundo conhece alguém que tem câncer. É uma loucura. É difícil ignorar que nossos hábitos alimentares e mudanças de estilo de vida causaram esse problema. O que você acha que está causando os problemas com a mitocôndria nas células cancerígenas? Talvez a comida, o estilo de vida, as toxinas, carboidratos em excesso, comida refinada e óleos vegetais?

Thomas Seyfried: Sim. Todas essas coisas que você listou. Não é somente uma coisa, mas tudo isso. Nossa agricultura agora é internacional. Plantamos comida em diversas partes do planeta. Essas comidas são enviadas para todos os cantos. A qualidade do solo… Os tipos de tratamento que são usados para tirar as pragas… A indústria alimentícia se tornou internacional. As comidas podem ficar nas prateleiras por dias, semanas e meses. Elas são nutricionalmente pobres de diversas maneiras. E elas desperdiçam muita energia. São cheias de açúcar. Tudo isso se acumula. É muito conveniente, já que é projetado para o consumidor. Quem são os consumidores? Nós somos os consumidores. A ciência alimentícia tornou essas comidas incrivelmente saborosas. Elas são cheias de doçura e são saborosas. Mas temos que entender como essas comidas são feitas. É um processo. O que esse processo faz com os nutrientes da comida? Juntando isso com o sedentarismo e o ambiente de trabalho estressante… o trânsito… as toxinas no ambiente… isso tudo causa uma inflamação sistêmica do corpo. As pessoas não acreditam que é possível passar dias sem comida, fazendo como faziam os antepassados. Nunca comíamos 3, 4 ou 6 refeições por dia no passado. Às vezes tínhamos um animal para comer e às vezes não. Hoje em dia, todos sabem que devemos comer a cada três horas – ou até a cada 2 horas. E que também devemos comer todos os grãos. Mas nos alimentarmos constantemente está causando um grande problema. Hoje nós precisamos até mesmo de um nome (jejum intermitente) para algo que já existia naturalmente. Foi necessário rotular esse hábito. Mas não é algo novo. Como você explicaria para as pessoas que não é necessário comer a cada 3 horas?

Thomas Seyfried: Isso é um choque para muitas pessoas em qualquer sociedade. Não se esqueça de que o ser humano evoluiu como um carniceiro, como os cães. Essas espécies comem e depois passam fome. Comemos muita carniça no passado. Encontrávamos um animal morto e o comíamos. Era mais fácil comer algo que estava morto do que ter que correr atrás dele. Então, evoluímos como carniceiros. Você tem que analisar essa evolução. A comida não estava disponível durante todas as estações. Nossa dieta mudava a cada estação de acordo com o que estava disponível. Não havia supermercado vendendo carne. Era algo sazonal. Algumas estações eram difíceis, sem comida. As pessoas tinham que se adaptar a isso. É por isso que temos a epidemia da obesidade hoje. Quando você começa se alimentar com comida real, seu corpo te diz quando você está satisfeito e isso regula seu apetite. É difícil comer com tanta frequência quando se come as comidas certas. Parece que as pessoas que gostam de saúde e não têm câncer usam mais a calculadora do que os pacientes de câncer. Eles não entendem isso. Rodrigo Polesso: Quando você para de comer com tanta frequência e começa um protocolo de jejum intermitente… Você pode parar de comer por 16 horas ou até menos. O que acontece com seu corpo? Muito se fala sobre autofagia. O corpo se livra dessas mitocôndrias imperfeitas e ajuda a prevenir o câncer?

Thomas Seyfried: Esse é um ponto importante. Não sabemos precisamente quanto tempo precisamos manter esse estado até começarmos a reciclar as mitocôndrias ruins. Podemos tornar as mitocôndrias que já temos saudáveis depois de 3 ou 4 dias de jejum de água. Mas em que ponto o corpo começa com a autofagia? Todas as células do seu corpo terão que funcionar em eficiência máxima, senão serão programadas para morrer. Os nutrientes nas células serão usados para alimentar as outras células. Rodrigo Polesso: Tratamentos normais que nós temos hoje em dia… eles basicamente matam nossas células junto com o câncer, não é? Eles matam o câncer, mas matam as outras células também. Talvez implementar jejuns e jejuns prolongados usando comida real sem ter que usar produtos químicos que te fazem sentir horrível por meses.

A estratégia atual para o câncer é levar a pessoa até uma cadeira para que ela seja irradiada. Elas não participam do gerenciamento da doença. Elas são simples observadoras. Por outro lado, na terapia metabólica a pessoa deve ser um participante ativo do gerenciamento da doença. Se eles não forem ativos, talvez devam ir serem envenenados e irradiados. As pessoas podem fazer escolhas. Elas querem participar no gerenciamento da doença ou preferem ser observadoras? Como você as convenceria a abrir suas mentes para dar uma chance às mudanças alimentares e de estilo de vida? Isso acontece com algumas pessoas que eu conheço. Elas estão recebendo tratamento para o câncer e confiam nos seus médicos. Elas acham que se algo desse tipo existisse, elas saberiam.

Thomas Seyfried: São as informações erradas. Os médicos não sabem disso porque não foram educados sobre isso. Uma coisa é dizer: “Eu usei terapia metabólica por anos. Sou um expert e sei que não funciona.” Nesse caso, você teria um bom motivo para isso. Mas quando você ignora uma modalidade terapêutica pelo fato de nunca ter ouvido falar sobre ela, foi porque você nunca foi treinado para entende-la… Quando dizem que os médicos nunca ouviram falar sobre isso, é verdade! Thomas Seyfried: Mas sempre é necessário um profissional que vai te ajudar. Você não pode fazer isso sozinho. Dietas cetogênicas são um processo médico. É uma ferramenta. Você precisa de um profissional que entende como aplicar a dieta, analisando seu sangue e fisiologia geral. É um processo. Você precisa de profissionais médicos treinados que te guiarão nesse processo. Nesse momento temos algumas pessoas treinadas para isso. Mas existe uma falta de conhecimento. A indústria insiste nesse sistema fracassado há décadas. Esse novo procedimento só é feito por poucos que entendem os mecanismos. Então, existe essa falta de conhecimento. Essas são mudanças fundamentais na maneira através da qual lidaremos com isso. Queríamos que isso fosse mais rápido. Não sabemos de que forma isso acontecerá. Estamos trabalhando em aprimorar isso. Estamos querendo provar o princípio e o conceito. Já estabelecemos nossos estudos e modelos pré-clínicos. É muito importante ter um bom modelo pré-clínico para o câncer. Existem modelos ruins que o campo usa. Daria para eu falar horas sobre como esses modelos são estúpidos. Eles tomam decisões sobre as vidas das pessoas baseando-se em modelos fracassados. Nós desenvolvemos uma metástase sistêmica nos nossos modelos. São modelos naturais de metástase. Elas representam a realidade. O que funcionar nesses modelos funcionará em humanos. Eles têm câncer no cérebro, câncer com metástase sistêmica (que são os mais agressivos). O que funciona nesses modelos funcionará em humanos. Gostamos de testar tudo no modelo pré-clínico antes de fazer recomendações ou de colocar em um humano. É assim que fazemos. Trabalhamos nos detalhes do modelo e depois ajustamos as diferenças de metabolismo e fisiologia entre as espécies. Isso pode facilmente ser feito.

Good afternoon, Professor. It’s a great honor to have you here today. How has your today been so far?

Thomas Seyfried: Thank you very much, Rodrigo. It’s a real pleasure to be on your show and have the opportunity to discuss some of our ideas regarding de origin, prevention and management of cancer.

Rodrigo Polesso: It’s a very big subject. I don’t see quality information being spread in the world, but in Brazil even less. So it’s a very good thing that we are having this chat today. Data from the American Cancer Society shows that more than 1600 people die from cancer per day in the US alone. Is that correct?

Thomas Seyfried: That is the number that they provide. Every year they come out with their overall assessment of the cancer problem. What I’ve done is catalog these numbers over the years. In general, what’s very interesting is that the number of new cases has increased dramatically – almost astronomically – in the United States. I think this is pretty much the same in most western countries. I think that is due in part to the better technology for screening and identifying cancer. But more troubling is the continuing increase in the number of people dying per year. If you look at the data for the last 4 years, the rate of increase of deaths is now faster than the rate of increase of new cases – the relative rate, not the numbers of people. Which is very troubling, because it says we are not making any major progress in managing the disease. People can argue with that, but the number of dead people speak for themselves. If there were a dramatic reduction in cancer, or a powerful new treatment, we would expect to see a dramatic drop in deaths, very similar to what we’re seeing in the AIDS crisis. Before the AIDS cocktails came around, AIDS was a lethal disease. People were dying in mass numbers, and then a therapy was developed. It doesn’t cure the disease, but it certainly led to a precipitous drop in the number of deaths. We don’t see anything like that in the cancer problem. Until we begin to see something like that, we have to ask ourselves what’s happening. What are we doing with all this research, money and investment? Obviously, something is seriously wrong here.

Rodrigo Polesso: Yes. That makes a lot of sense. We usually talk about obesity here. I think it’s the same thing. We see new diets, treatments and medicines coming out, but we don’t see the levels dropping at all for any markers of metabolic disease decreasing. I believe cancer is the same. The main paradigm right now states that cancer is a genetic disease and just a matter of bad luck. The new paradigm that you are in the forefront of, is that cancer is mitochondrial metabolic disease. I think it is very interesting and it is very difficult to ignore this type of things. It is not genetic in the great majority of the cases. Instead, it is caused by the way we eat and the lifestyle we carry. What can you say about it?

Thomas Seyfried: I think that is right. We have been led down a path of misinformation for so many decades now. One has to go back and look at the historical perspective on how that happened; the view of cancer as a genetic disease. Right after the Second World War there have been a lot of work on the virus induced cancers and a variety of questions: how does this disease arise? Is it an infectious agent? Is it a virus? I think the discovery of the DNA structure had such a profound effect on the mentality of science and medicine at that time. It was almost as a tranquilization of the mind. People just ran off in a stampede. If you look at gene expression in cancer, of course it’s abnormal. Almost all the entire scientific field is looking now to bring a science of medicine and biology down into the molecular level. There is also been a envy of physicist in science for centuries. Physicists have always been recognized as the pure thinkers of the natural world. Biology has always been very vague, very difficult to define. There are too many variables. It wasn’t as quantitative. The biochemists were bringing it down. But when you find gene mutations at the molecular level that now maybe playing some role… it was just so exciting to think that we can take this very ambiguous field of biology related to medicine and cancer and understand it at the molecular level. Now we become more molecular, more precise, more physics-like. But nothing could be further from the truth with the respect to the origin of cancer. This is a human disease tied to our own societies, to the foods that we eat and the kinds of life styles that we have. This is every bit as much of a personage kind of a problem and not only changes at the molecular level. Now we have seen this morph into an industrial wide approach to this disease, where all the new therapies are based on some molecular changes, which have enormous financial benefits for those who are participating in this activity. The research grants are awarded to those individuals that are looking at cancer at the molecular level. Much less is given to other aspects of our behavior that predisposes us to the disease or even to nutricially manage the disease. These are less quantifiable but nevertheless extremely powerful in how we are going to address the disease. So, we have a major issue here. We have a massive misunderstanding of what the nature of this disease is. It is not a nuclear genetic disease. It is a mitochondrial metabolic disease. Once this becomes more understood, at the biochemical, molecular and physiological levels, only then will we begin to see a shift in the direction where eventually we will drop the numbers of dead people just as precipitously as it was done for the AIDS epidemic.

Rodrigo Polesso: Yes. It seems that people run around the mechanisms of the mutations. We all know that cancer has all kinds of mutations, but they all have something in common: the way the cancer cells work with glucose. All the cancers cells can’t breathe properly. Their respiration mechanism is kind of broken. So they all ferment for energy because of that. That happens in all the cancer cells. That is something we know they have in common. That gives us a little bit of a hint of what the cause is.

Thomas Seyfried: Yes, that is very important. That is a very clear idea of what the problem is. Every cell in the tumor is a different genetic entity. They have hundreds of thousands of mutations. All different kinds, drivers, passengers, initiators, facilitators… they name all these different classes of mutations. But you are right: all the cells are fermenting. How aggressive the tumor is linked to how much it ferments. The most aggressive tumors are the greatest fermenters. The most indolent (slowly growing tumors) are partially fermenters. No matter how you look at it, why are the tumor cells fermenting? If you are part of the gene dogma, you will say that the tumors cells are fermenting because they have mutations in oncogenes that make them ferment. On the other hand, if you have damaged respiration, they are fermenting. We know that when we stop breathing, lactic acid builds up in our body because our cells begin to ferment – they can’t get oxygen, so they throw out lactic acid, just like the tumor cell. The cancer cell is locked into this state of fermentation. So, what kind of genetic changes would occur to lock in a cell into fermentation? That is these oncogenes. The oncogenes are essentially facilitators of the fermentation metabolism. But, if you allow those cells to respire, the oncogenes turn off. So, clearly, the oncogenes are an effect, they are not the cause of the disease. They are facilitating the ability of that cell to stay alive because it can’t respire effectively. If it doesn’t respire effectively, it either ferments or dies. So, these oncogenes are facilitating this locked in metabolism. They are not the cause, because if you give them new respiration, the oncogenes turn off and they become normal again. So clearly, they are responsive, they are not the cause. They are the effect! Yet, this simple concept is almost impossible for the scientific field to understand. They will find every kind of exception based on a flawed experimental system to hold on to the idea that this is a genetic disease. The late people that will make the change, not the top guys. They are locked in to this. They are locked in to the dogma. And it is very hard to break the dogma. So, when I look at cancer as a mitochondrial metabolic disease with metabolic solutions, who would like the solutions? The cancer sufferers! The patients would like this. So the patients will put the pressure on the establishment to make the chance. Because the establishment’s solution to this problem is a failed one. It is based on a flawed scientific concept.

Rodrigo Polesso: It is scary how similar it is to nutrition in general. You probably know that, of course. The dogma is just impossible to break. I think it will take a generation to start changing. And the old guys start fading out, the new generation will not be able to ignore it. I think people that are getting real results out of nutrition are getting real results focusing on the cause of cancer that you talked about. They are getting real results, so I think they will create a demand for this. People won’t be able to ignore all of this. For people that are new to this concept of cell fermentation, I think this is key to everything. We are talking about to fill the cancers cells. Can you just explain to people that are new to it how a normal cell processes energy and how a cancer cell processes energy? Just so that people can understand better the differences.

Thomas Seyfried: I think that a good way to address that question is to think about the origin of life on the planet. There was a time on this planet when there was no life. There was nothing. Gradually, biological organisms appeared 2 to 3 billion years ago. The first kind of organisms that appeared on our planet; there was no oxygen in the environment. The oxygen had yet to come in to the environment. Yet, we had living organisms on the planet before oxygen came into the atmosphere. How did these cells live without oxygen? They fermented. They used a very primitive form of energy metabolism. They were not using oxygen as a final exceptor of electron because there was no oxygen. So they were using other organic molecules like amino acids and certain kinds of primitive proteins. Every organism at that time was fermenting. Fermentation is the use and development in the absence of oxygen. So, all the organisms proliferated uncontrollably as long as there were fermentable fuels in the environment. When the fermentable fuels became limited, those organisms were dying. When oxygen gradually came into the environment, some 1 and a half billion years ago, there was some organisms that developed that could capture and use oxygen in a primitive way to get energy. And then there was this hibernization between this oxygen capturing organism and these fermentable organisms. There was a symbiosis – a working relationship – between the organism that could capture oxygen and this powerful fermentation machinery in the organisms that fermented. Development in evolution showed that with oxygen you could generate a lot more energy, much more efficiently, without using so much resources. We gradually developed from single cells into multi cells. Because now we have energy efficiency, these cells could do more sophisticated functions, branching off to do a variety of different things. Rather than a body in a single cell, we then had organs and tissues and all these different cells could then work in a much more energy efficient way to accomplish a much more sophisticated environmental biology. Now we have mammals and all these other organisms that we have now on the planet that evolved from these primitive organisms. What happens in cancer is that there is a damage to this symbiotic relationship within the cell. Interestingly enough that little bacteria that was able to capture oxygen as a separate entity relinquished a lot of its genes to this fermentation organism. But, that little bacteria that uses oxygen maintained 13 genes that are absolutely critical. Those are the kill switch. If any thing happens to any of those 13 genes, the whole cell dies. But they were retained in the mitochondria. The mitochondria today is the result of that energy capturing organism. So it arose from a bacteria. Yet it allows ourselves to do all these magnificent functions, facilitating and entire body physiology. What we have then, is a damage of that organelle. As Warburg said, if the organelle and respiratory function of the cell is critically damaged the cell will die. It cannot adapt and adjust to an acute loss of energy. What cancer is a protracted, prolonged, slight injuring of that respiratory capacity. It is a chronic problem. It is not an acute problem. What that does that, is that is allows this cell to gradually transition its energy from respiration using oxygen to fermentation using organic molecules and existed in all of our cells. That primitive pathway is still present from before oxygen came in on the planet. These cells now fall back into a state that existed prior to the presence of oxygen, which was uncontrolled growth, rapid proliferation and everything we see in the disease today. So the solution to the problem is targeting the fermentable fuels that these cells are using and then transitioning the whole body off to respiratory fuels that the cancer cells can’t use. It is a strategy based on the origin of the disease and it should be very effective once we fine-tune it and work out the kinks.

Rodrigo Polesso: Right. The main fuel for fermentation would be glucose, is that correct?

Thomas Seyfried: Glucose and there is also amino acids. A lot of these pre-oxygen microorganisms could ferment carbohydrates as well as amino acids. So we always have to recognize that there will be some amino acids that could be fermented as well. But the most abundant fermentable fuels are glucose and glutamine. These are the two fuels that the cells cannot survive without. Targeting those two fuels simultaneously should be able to eliminate the majority of cancer cells. Every kind of tumor are all the same. There are not all different diseases – it’s a misconception. As you said in the very beginning they all arise from different tissue, they have different gene mutations, but they are all fermenters. Regardless if it is a brain, bladder, breast… whatever it is, they are fermenting. So, what are they fermenting? They ferment glucose and glutamine. So the solution to the problem is getting rid of glucose and glutamine, or targeting in very innovative and sophisticated ways. Who is doing this? No one is doing this.

Rodrigo Polesso: Right. Some people are doing this kind of thing. Not in a big scale. I saw some animal studies. When they reduce the glucose in the food, get people in ketogenic diets, or even some prolonged fasting… We cut the fuel supply to these cells. A normal human cell would survive without the glucose burning ketones and other fuels. But the cancer cells would have a problem with that, right? If you cut the glucose and the glutamine, like you said, what happens to the cancer cells? Do they starve to death or weaken? Some people say there is no such a thing as starving the cancer.

Thomas Seyfried: It hasn’t been well thought out. We are doing that right now as we speak. The issue is that many people are targeting glucose through ketogenic diets and a variety of glucose targeting drugs. And you are absolutely right. The body can switch to an alternative fuel: ketones. But the cancer cells can also switch to an alternative: glutamine. So, if you have a tumor that is glutamine dependent, then you are not gonna get full therapeutic benefits from a ketogenic diets. The key thing that we have defined that is not very appreciated is that the metastasis – the spread of the tumor throughout the body – is the singular greatest threat that we have when we are diagnosed with cancer. If you have a lump and that lump has not yet metastasized, the surgeon can easily cure a cancer. But if the cells in that lump decide to spread, then it becomes a much more difficult disease to manage. What are those kinds of cells that leave this primary tumor and spread throughout the body? We and other have shows that these are part of the immune system. These are macrophages. What do macrophages eat? They eat glutamine! They use glucose or glutamine and they can live in hypoxic environments. This whole concept of antiangiogenic therapies is only superficial. The cells can already live in hypoxic environment. So, antiangiogenic therapies like Avastin, are not gonna work. They are only gonna have a superficial effect on the disease. Once you understand the biology of the disease, you can almost predict. I don’t have to waste a lot of time looking at different cancer therapies to know whether they are gonna work of not. They are spending millions and millions of dollars looking at these drugs. I already know whether they are gonna work or not. If they are directed to the metabolism of the cell, based on what I just said, they will know right off the bat whether they are gonna work or not. There are a number of drugs that are available that could target both of these molecules, but they off-patent, people aren’t using them. It is just not “exciting”. To the patient cancer who has lung cancer, they don’t care if the research is exciting or not. They wanna know: “what do you have for me?” They are also not willing to wait around for the people that hold the gene theory to die off. They have something that is an immediate crisis to their body. They want something that is gonna keep them alive. And they don’t wanna wait 50 years for the guys that are locked in to these gene theories to die off. Because they are gonna be dead before them! You have to look at the seriousness of what we are dealing with here. We are dealing with the lives of millions of people. We don’t need to wait for such a long period of time when we already have the answers at hand. We just have to focus on top research labs doing this. They are all looking for the patentable drug. The drug that can do this. But we already have drugs and procedures already available – it’s just that you are not gonna make a lot of money on those. But I don’t think the cancer patient cares about that. You have to always go back to the patient. What does the patient want? The argument is always: “there is no clinical trials to support these kinds of metabolic therapies.” Who is gonna pay for these clinical trials? Does a big drug company wanna step forward and use a therapy when there’s no patentable drug that can be developed from this? And the we have to do double blind cross-over experiments and all these kinds of things that you would have to do if you were proposing to use a toxic drug. This is nonsense. We have to rethink the entire way we are treating cancer patients. In other words: our strategy, which is called press-pulse metabolic therapy, where we put patients on these different metabolic states and pulse them with low dose drugs that work together with nutrition and diet. They are all working together in a synergistic interaction. Patients should emerge from this therapy healthier than when they started. They shouldn’t be destined to have these medical abnormalities as the result of being treated for their cancer. This is nonsense! This is nuts! This is a further statement to the absolute failure of our approach to this disease. Something has to be done and has to be done soon. Why should we wait around for all this when we have the strategy at hand?

Rodrigo Polesso: It’s difficult to believe that this happens. If Ebola was killing 1600 people per day it would be a world chaos. Everyone would be talking about it. That’s what happens with cancer. But there’s nothing you can do about, it’s just bad luck. What kind of results do you see by people that just take matters by their own hands and try to implement these strategies by themselves? Even without the drugs, but just implementing a protocol of intermitting fasting, long fast and ketogenic diets. Do you see remissions, improvements, cure?

Thomas Seyfried: We don’t like to use the term “cure”. We can’t know that. The field uses the 5-year mark. But there’s a lot of people that live 5 years and die after 6, 7 or 8 years from the cancer that they had or some other cancer that developed as the result of the treatments they received. This is nonsense. How do you know you have a cure? If you live to be an old person and you die from something other than cancer. Then you would be definitely cured from cancer. How do you manage the disease, keeping the patient healthy, with a good quality of life and progression free survival for long periods of time without any adverse effects from the treatment? This is the strategy that we have. This is the goal that I think we would like to use. Most of the case are what we call “anecdotal”. Anecdotal cases mean “it hasn’t been done by a clinical trial, but that guy looks like he’s awfully healthy”. Unfortunately, the medical establishment rather than helping patients adapt do metabolic therapy, usually discourage people from doing this. Often times they will not be supportive of a metabolic therapy until the patient has gone through this toxic standard of care, of mutilating surgery, radiation and poisonous chemicals. Once the patient fails all this, then maybe they’ll consider this possibility. It’s not entirely the fault of the field. They are simply not trained in medical schools to know how to use food as medicine. They are not trained to study a whole physiology metabolism in targeting cancer. Cancer is viewed as a focal disease in some organ or tissue. It is not viewed often as entire systemic disease. Once you realize it is a systemic disease, happening to involve a breast or lung, then you must treat the entire physiology of the patient. And, at the same time, degrade de tumor slowly, using natural therapies, that enhance the health and vitality of the patient, rather than reducing the health and vitality of the patient. This is a strategy. This is the press-pulse strategy that we are presently working on. Dominic D’Agostinho and myself are working on this new strategy. It’s a framework by which we’ll be able to manage cancer effectively is a slow, manageable process that will lead to a greater state of health and either the elimination of the tumor or bringing it down to a state that is no longer a threat to the persons life.

Rodrigo Polesso: That’s great. That’s great news. Very good to hear that. Please correct me if I’m wrong, but a few hundred years ago, we didn’t see cancer as a very common disease. I think it was kind of a rare thing. Maybe only the real genetic cancer happened back then – which was a rare thing. That’s what I see in studies – it was a rare condition. Nowadays is a very common condition. Everyone knows someone that has cancer. It’s pretty crazy. It’s difficult to ignore that our food habits and lifestyle changes are causing the problem. What do you think are causing the mitochondria problems in the cancer cells? Do you think it’s more the food, lifestyle, toxins, excessive carbs, refined food, vegetable oils?

Thomas Seyfried: Yes. Everyone of those things you’ve just listed. It’s not of thing, it’s the whole bag of them. We have moved now to international agriculture. We grow foods in all different parts of the planet. They are shipped all over the world. The quality of the soil… the types of treatments that are being used to keep the bugs away… The food industry itself has become international. The foods can remain on shelves for days, weeks or months. They nutritionally deplete in various types of things. They are very high in wasted energy. Very high on sugar. You put together all these kinds of conditions. It’s also convenient. These are all designed for the consumer. Who is the consumer? We are the consumer. Food science and technology have make these foods incredibly tasty. Full of sweetness, goodness and tasty. But we have to put together where all this food comes from. It is a process – what is it doing to the minerals and nutrients in the food? Together with less exercise and the work place… communing in cars… toxins in the environment… you put it all together and what we have is a systemic inflammation in the body from a variety of assaults. That will lead and predispose us to cancer. Cancer cannot happen in a cell that has healthy respiration. Cancer only happens in cells that have damaged respiration. How do you protect yourself from damaged respiration? Therapeutic fasting. But it’s very hard to do! Most people on the planet don’t wanna do this! Who does? I don’t wanna do this! “I gonna stop eating for a week so I don’t get cancer? Is there another way?” Ketogenic diets can do it. But a lot of people don’t like ketogenic diets. There is no carbohydrate, it doesn’t taste good. If you can find a ketogenic diet that does taste good people say: “I will eat more of it and see if I can get healthy!” No, you will get insulin insensitivity and dyslipidemia. So you have to have people that understand what is going on here. I think it’s a balance. We cannot deprive ourselves of the technological advances in food, drink and all the other things in our diet lifestyle. But we have to know that they also put us at risk for developing cancer. But if we can develop a habit of moderation… But human beings are not moderate. They are extremists in every way. They either do one thing or the other to the maximum. The moderate guy is a minority on the planet. I don’t what society you are in. The moderate guys is like, you know… We always have to adjust that. Cancer was unknown in primitive societies. Eskimos, jungle tribes in Africa or even in the Amazon… they don’t even have a word for “cancer”. It only became a play as our technology and food and science advanced. All of these things come together to put us at risk. Another thing you mentioned about the real genetic cancers… Those are also secondary effects. This is a difficult concept for people to understand. We know that BRCA1 mutation causes cancer in some people, just as the P53 mutation. Those mutations encode protein in or affect the function of the mitochondria. If we look at the whole spectrum of the cancer-causing agents… radiation, chemical carcinogens, virus… All the origins that have been found impact on that function of that organelle. All of those causes are secondary. The primary cause is damage to the respiration of the cell, which can be caused by a chemical, a virus or an inherited gene. So, that doesn’t make cancer a genetic disease, it is still a mitochondrial metabolic disease. It just happen to have an initiating factor coming from the genome. There could be another person that has the cancer who initiated in from a chemical. But they both share mitochondrial defect. How do we know those rare mutations are now the origin? We rarely find a cancer that is 100% penetrate. Some people with BRCA1 mutation never get breast cancer. They only get cancer with the mutation actually disrupts the function of the mitochondria. So, again, the mitochondria becomes the prime organelle and the origin of the disease. Mitochondria destabilization is the origin of cancer. No question about this. This is the origin of the disease. If you wanna prevent the disease, you prevent your mitochondria from becoming damaged. How do you do that? You take diet and lifestyle conditions that will keep your body in a healthy metabolic state. It is very unlikely that you will ever get cancer if you can maintain that. As you age, more factors in the environment cause damage to you mitochondria. Older people get more cancer than younger people. It becomes completely understandable as you view it as a mitochondrial metabolic disease. That gives you power to manage the disease.

Rodrigo Polesso: I think we can say that cancer is probably another branch in the tree of metabolic diseases caused by the very same things: obesity, diabetes, Alzheimer’s, cancer. All of them are caused by the very same thing. But nobody focuses on the cause, which is our life style and the food we are eating. When you see the research and analyze it with an open mind, it is very clear that something is very wrong. These wrong habits happened very recently in human history. All of these diseases were pretty much extinct or non-existent back then. Something happened in the last few decades or so that made all the disease go up together.

Thomas Seyfried: You are absolutely right. There were some regulatory changes in the United States which allowed high-fructose corn syrup to replace glucose in the food industry. This became very profitable. Everything has to be the profits. The food industry, the chemical industry… all these industries. Their profits are what drives these technologies. Unfortunately a side-effect of all these profits are creating an environment that predisposes human beings and dogs.

Rodrigo Polesso: The dogs. That’s true.

Thomas Seyfried: Cancer is dogs is out of control as well. What are they eating? They are eating processed foods just as we are. They don’t eat well, they don’t exercise. They sit in somebody’s apartment all day long chowing down in some grain food, and they end up with cancer. What’s the big surprise here? It’s the same thing in dogs. In dogs we have direct evidence that if you change their diet and life style, you can actually cure their disease. The Keto Pet Sanctuary is proving this. I have seen it myself. It is unbelievable how powerful food is in curing dog cancer.

Rodrigo Polesso: You mentioned that someone went to veterinarian and they said they “don’t believe in feeding raw meat to the dogs”.

Thomas Seyfried: That’s true! That was a policy issue for the Vetenarian Society of America. I don’t know if that was influenced by the dog food industry. What are they going to do with all these bags of dog food in the dog food aisle? It is so convenient to people to come home and pour a bunch of food into the plate of the dog. The dog will eat anything, let’s be honest. He won’t say “this food is no good for me, I don’t wanna eat it”. The dog will eat anything. It’s very convenient. Rather than going to the butcher and chopping up a chicken, and throw the pieces of the chicken in the dog plate. It’s just much easier. But the dogs are getting cancer as the result of it. They are getting all the same afflictions as we have. It is very clear to me – and it should be to most people with a few functional brain cells – what is going here? It’s not that much of a mystery.

Rodrigo Polesso: Let’s talk a little bit about something that can help people that have cancer. You talked about calorie restriction. You said something in a recent talk that I thought was brilliant: “Human being are evolved to starve.” We are told everyday that we need to constantly feed ourselves every 3 hours. People have a hard time believing that we can go days without food, easily mimicking food scarcity throughout history. We never had 3, 4 or 6 meals a day during history. We never knew… sometimes we’d kill an animal and sometimes we didn’t. Nowadays, everyone knows we should eat every three hours – or even 2 hours, who knows. And that we should eat all the grains and everything. But constantly feeding yourself is causing a big problem. Now, we need a name (intermitting fasting) for something that already existed naturally. Now we need to label this habit. It’s not a new thing, it’s an old thing. How do you explain to people that they don’t actually need to eat every 3 hours?

Thomas Seyfried: Well, that’s gotta shock many people no matter what society you are in. Don’t forget that human evolved as a scavenger as well, like the dog. Scavenger species are feast and famine. Our history was scavenging. You find a dead animal, you eat it. It was much easier to eat something that was lying dead there, than having to run around to kill it. So we evolved as a scavengers species. You have to look at our evolution. Food was available during seasons. Every season our diet would change depending on what was available. We didn’t go to the supermarket to get beef every week. It was seasonal. Some seasons were harsh, there was no food. People had to adjust and adapt to this. This is why we have the obesity epidemic today. Most of our evolutionary history was of energy conservation. So, our bodies are remarkably efficient at storing energy. Because that would allow us to function for long periods of time without food. The obesity epidemic today is a direct consequence of our evolution as a starved species. Now, for the first time, we have this massive amount of high caloric food available. And the body has this hard-wired ability to store energy. What do you think the outcome is gonna be? All this is perfectly understandable in the light of evolution. When people don’t understand evolution, this all becomes a mystery. In the light of evolution, this is all perfectly explainable and understandable. The question is: what do you do about it? Most people treat the symptoms, whether it’s obesity, Alzheimer’s disease, cancer. We are treating the symptoms rather than the origin. I think we need better education. And we need a gradual movement, a change. The food industry maybe the spearhead. They cause a lot of this in one way or another. But they can also resolve a lot of this. As long as there is money to be made in the paradigm shift. And at the same time educate people. It’s gonna be a transition. Unless people just accept cancer as part of our progress. I don’t feel that way.

Rodrigo Polesso: I agree. When it comes to eating frequently, when you change the quality of what you eat, the quantity adjusts itself. When you start feeding yourself with real food, your body tells you when you are satisfied and regulates your appetite. It is difficult to eat as often or as much when you eat the right foods. That is true for obesity and probably for cancer. Eating the right foods and having the metabolic system working properly, it is probably easier to implement an intermittent fasting protocol as well. A normal person, nowadays, if they implement a real food diet and an intermittent fasting protocol, can this type of approach prevent cancer or even help people that are going through a treatment?

Thomas Seyfried: I think it could. We developed a glucose-ketone index calculator. We published it in Nutrition & Metabolism. It seems that people who are more into health care, who don’t have cancer, seem to be using the calculator more the cancer patients are. They don’t understand it. If you think cancer is a genetic disease, why are you measuring blood glucose and ketones? People who are interested in improving their health and metabolism and fasting, can use this glucose-ketone index calculator. When an amount of ketone bodies that enters the blood stream is equal the amount of glucose, then you are in therapeutic ketosis. That’s when the cells in your body are burning ketones, which don’t produce reactive oxygen and enhance the health and vitality of the mitochondrial cells. So, if people are going to change their diet and lifestyle, it would be good for them to use the calculator, because that gives them at least some guidelines. It gives them a biomarker to know whether or not they are actually enhancing the health and vitality of their mitochondria. That’s one of the aspects of the glucose ketone index calculator. People can use that. They can say: “What’s my ketones today?” If it’s very very low and your glucose is very high, you are not in a good zone. But if you stop eat for three days… We have done in the lab with my students. Young people can get down to these metabolic zones very quickly. Within three days they have very low glucose and very high ketones. But you don’t have to stay in this state for very long. Just a few days has remarkable health benefits to the patient. Of course, younger people can get to these zones faster than older people. The older person has been more habituated to high glucose foods. To go off the high glucose foods and into a state of therapeutic ketosis is much more difficult. It’s like an addiction to glucose. Getting off glucose has the same discomfort as getting off alcohol, cigarettes or drugs. It’s hard! Some people get sick, get light headed, they feel terrible. They don’t wanna do it anymore! “What am I doing? I’ll go back to eating the big burger.” It’s not easy. All these things are not easy.

Rodrigo Polesso: There is an adaptation period, right? We used an analogy here before. If you go to the gym and you never exercised in your life, tomorrow you’ll soar everywhere. But you don’t see any muscle growth. So you immediately conclude that exercises don’t work.

Thomas Seyfried: “Exercise doesn’t work and it hurts!”

Rodrigo Polesso: “It just causes pain!” It’s the same thing. People have to go through an adaptation period. You mentioned a calculator – where can people find it?

Thomas Seyfried: It’s an open access paper. Open access means anyone who has a computer can get into the journal. It’s called Nutrition & Metabolism. It’s an open access journal at BioMed Central. In there, on the back of the paper we have a little figure that you push the button to it and the calculator immediately comes up. It does everything. All you have to do is get a meter to measure your blood from a finger stick. You can get a drop of blood, like people with diabetes. There is a meter you can get from Amazon from 39 dollars or something. It’s a glucose ketone index meter. It measures glucose and ketones. You have to do it in blood, not urine. That doesn’t work, it’s not accurate. You need the blood glucose and the blood ketone. And then you can keep accurate records every day. You can see your blood sugar going down and your ketones going up. You’ll have biological evidence that you entered the therapeutic zone of ketosis. At least the person will know that they are in this zone. The longer they stay in this zone, the healthier the mitochondria will become and the less likely will be getting cancer, diabetes, Alzheimer’s disease or cardiovascular disease. All these chronic diseases are very much reduced when you are in therapeutic zone. The question is: “How long must I stay in this zone?” If you can stay there for 2 or 3 days every 6 months, it’s gonna have remarkable health benefits for you. Before the calculator, we had no way of knowing this other than thinking we were in the zone. Now we have a biochemical marker to tell us when we are in the zone and how long we’ve been in there. It’s a very simple tool. It doesn’t cost much. It’s very helpful. Go on the web and you’ll see these guys doing water only fasting for 5 or 10 days. They measure their blood glucose ketones. They tell everybody how they feel and what’s like.

Rodrigo Polesso: When you stop feeding yourself every so often and start with a intermittent fasting protocol… You can stop eating for 16 hours or even a shorter period of time. What happens to body? We talk a lot about autophagy. The body probably gets rids of these imperfect mitochondria and helps prevent cancer?

Thomas Seyfried: That’s an important point. We don’t know precisely how long we have to maintain this state of inanition before we start recycling bad mitochondria. We can make the mitochondria that we have very healthy after a 3 to 4 day water only fasting. But at what point does the body start using internal surveillance (autophagy)? Now every cell in the body must work to maximum efficiency, otherwise it will be programed to die. The nutrients in that cell are used then to feed the other cells in the body. Everything in the body will be efficiently used. This brings us back to that very energy efficient state. How long do we need to maintain this? These are very important questions that have done ben fully evaluated. But we have a pretty good idea. Seven days should start the process of autophagy. But how many people could stop eating for 7 days? This is not easy. Will it happen if you go into therapeutic ketosis, if I take a small amount of a ketogenic diet? Will that induce autophagy after 7 days and take the sting out of this therapeutic fast? I say “sting”. I think most people would be uncomfortable by not eating for 7 days. Some people would go crazy. People would think this person is crazy. You are gonna get a lot of push back, blow back and this kind of stuff from other members of the community. But you are doing for a particular reason. Sometimes it’s a challenge. “Can I go 7 seven without eating?” The answer is: “Of course you can.” It’s like climbing a mountain. But do both to see which one is easier, climbing a mountain or not eating for 7 days.

Rodrigo Polesso: Normal treatments that we have nowadays… they basically kill our cells along with the cancer, right? They kill the cancer, but they kill all the other cells as well. Maybe implementing intermittent fasting and long fast with real food or going through a bunch of chemicals that make you feel like crap during months of your life.

Thomas Seyfried: When you do the metabolic therapy, the fasting is just one part of the overall strategy. There are certain drugs that work very well together with that kind of fasting. Other drugs from the pharmaceutical are poison. You don’t have to do this to anybody. You don’t ever poison somebody to make them healthy. This is insane! Why are we doing this? This is nuts! If you can achieve the same goal, which is to manage cancer without the poison. This can happen! So, why are we poisoning all these poor people to get rid of their cancer? Their hair falls out. Why would somebody’s hair fall out while being treated for a disease? If you go to the doctor and say you have a little indigestion and the doctor gives something that makes my hair fall out, this is crazy! But if you have cancer, this is expected. Not unexpected. Having your hair fall out is just one of them. You can even die from the treatments. You should never have to be treated this way. This is a state of profound ignorance of what the nature of the disease is. If you understand that it is a metabolic disease and you don’t have to pay one group of health cells for a group of 6 cells, and you feed the healthy cells, making them stronger. Then you make the cancer cells weaker and weaker to the point until they are absorbed by the normal cells. This is the strategy. You need an innovative kinds of strategy that will help a lot of people. Unfortunately, some other people will have to be part of this. The current strategy for cancer is that the poor person goes, sits on a chair and gets irradiated. They are not participating in any significant way in managing their disease. They are a bystander in this process. On the other hand, in the metabolic therapy the person must be an active participant in the management of the disease. If they are not an active participant, then hey should go and get poisoned and irradiated. People will have a choice. Do you wanna participate in the management of your disease or would you prefer to be a bystander in the management of your disease? Right now, everybody is a bystander. “We didn’t do the clinical trials.” We have plenty of anecdotal evidence that it will work if it’s done correctly. We have people educated and trained to do this. The field has to make a major change. I think it’s gonna come from the late people, not from the top medical schools.

Rodrigo Polesso: Now for the most difficult question. If someone currently has cancer, and it’s being treated by the conventional therapies, trusting wholeheartedly if there was a better way of treating the disease, their doctors would definitely know about it. How would you go about convincing them to open their minds to give a shot to this dietary lifestyle changes? That happens to some people I know. They are getting cancer treatment and they trust their doctors. So, if such a big thing existed, certainly their doctors would know about it.

Thomas Seyfried: It’s just misinformation. The doctors don’t know about it because they have never been educated about it. It’s one thing to say: “Yes, I have used metabolic therapy for years. I am an expert at it and I don’t think it works.” Ok, then you have a very good valid reason for that. But when you discount a whole field of therapeutic modality, based on the fact the you never heard about it, and you have never been trained to understand it… When they say their doctors never heard about it, it’s true! How can they hear about it if they are not trained to know about it? Unless they go out of their way. This is what we are seeing. More and more physicians who are investigating these things on their own, talking to people, going to meetings… They are becoming mavericks on the field. On the other hand, it’s very difficult because the AMA holds the procedures that are supposed to be given to various diseases. If you don’t follow the standard of care, of what has been recognized as important, you could lose your license. So they have a lock hold on what needs to be given. What is their role in this whole thing? Why are they not participating in the education? Why are they bystanders in this whole thing? It’s a reeducation of the entire system. What I am saying is supported by clear data from the scientific community. This is not some fictitious state that I am making. I have looked at the data. I have seen it working in people. I have seen it working in our pre-clinical models. Is it perfected yet? No! But can we perfect and fine-tune it? Absolutely. Will it get better? Absolutely. Will it be the solution to this problem? Yes. It’s just a matter of time. It just has to be understood. We are doing what we can do. I think more and more people are coming to realize… What point at the diagnosis do you initiate metabolic therapy? You go to the physician and the prescribes the standard of care, which is usually some concoction of poisonous chemicals and radiation. If that doesn’t work, you are free to do what else you want. This is nonsense. It’s the same thing with epilepsy. I’ve worked in the epilepsy field for years. We have very powerful drugs to measure epileptic seizures, but they have significant adverse effects. A child that has epileptic seizures and receives metabolic therapy can have the seizures managed in the majority of cases. Why would they wanna use these toxic drugs if we can manage the seizures with a metabolic therapy? We should do the same thing in the cancer. If we have someone who has cancer, that person should have the press pulse therapeutic design to see if they can manage their cancer through fasting, drugs that work with fasting, ketogenic diets, ketone supplementation, hyperbaric oxygen… all strategic applied to this patient. We have never seen a tumor not respond to this. If the tumor don’t respond as effectively, they are always free to go back and get poisoned and irradiated. They always have that option. The current standard of care poisons and irradiates de cancer patients, making their body very weak. Many maladies and current because of that. Now we are gonna go on a very vigorous metabolic therapy that requires your healthy cells to be very healthy. Now they are not healthy because of the previous treatment. That makes it worst, because it produces a toxic effect in your body. We want no inflammation. We want a person with reduced inflammation. Radiation and chemo create a massive amount of inflammation. They free up glucose and glutamine – the two fuels that are driving these tumors. It creates a completely different metabolic system. And consequently a lot of people suffer and die as a result. This is needless suffering and dying. Not enough people know about these alternative therapies. They are not quackery. This is real hard science. This is what the fundamental issue of biochemistry and health is actually about. The fact that the medical community is unaware of this is a tragedy in itself.

Rodrigo Polesso: Absolutely correct. And there is no downside in trying it out. If you have cancer and started the other therapies, there is no downside in eating right. There is no downside in prioritizing real food.

Thomas Seyfried: But you also need some professional that is going to guide you and help you. You just can’t do it on your own. Ketogenic diets are a medical process. It’s a tool. Anymore you would go and inject yourself with a toxic cancer to reduce your cancer. You can die from this. So, you don’t go home and do yourself a ketogenic diet. You need a professional who understands how to apply the diet, in light of your blood work, in light of your overall physiology. This is a process. You need trained medical professionals who will guide you through this process. Right now we have very few of these people that are actually trained to understand this process. So we have this gap of knowledge. We have the industry on one side that has been tread milling on this failed system for decades, and then we have this new evolving procedure that only a few people know how to implement it, or even understand the underlying mechanisms. So we have this massive gap of knowledge. We have to fill that gap with knowledgeable people. Of course, the enthusiasm should come from the patients. If the patients knew they could work with a trained professional who is going to guide me through this metabolic therapy, keep my body health and degrading my tumor… They would love to work with that person. These persons should be compensated. They are health care professionals just as much as the person doing the radiation. They are very well compensated people. They are incinerating a part of the body and potentially creating an environment that is gonna lead to the recurrence of that tumor or make you sick for a variety of other reasons. We have to change this whole process. These are major fundamental shifts in the way we are gonna deal with this. We’d like to see it happen quicker. Who knows how it’s gonna happen. We are working to fine tune this whole thing. We are working to prove principle and concept. We published our studies and pre-clinical. The most important thing is to have a good pre-clinical model of cancer. Not the bogus models that the field are using. I can go on about how stupid some of these models are. They are decisions for people’s lives based of failed model’s systems. We developed the old natural systemic metastasis in our systems. They are natural models of metastasis. They represent the real world. What works on these models will work in humans. They have a brain cancer, we have systemic metastatic cancer, which are the most aggressive tumors. What works on this model will work in humans. We like to vet everything in the pre-clinical model before making recommendations to put on any human being. This is the way we do it. We work out all the details of the model and then we just adjust it for the differences and basic metabolic rate physiology between one species and another. This can be easily done.

Rodrigo Polesso: That’s how it should be done everywhere. Professor, I really thank you for your time. I also thank you for your work. I hope you continue strong. I hope this information reaches people that need to get in contact with it. It can really help a lot of people. We usually talk about nutrition and obesity and it’s very good to know that the same causes are also influencing cancer in people. There is a bright light at the end of the tunnel – even though it’s a hard light to see… I really thank you for your time. Thanks for participating in this podcast today.
